Interested in #ACP (care planning) in Scotland? Our project 4ACP ed.ac.uk/usher/primary-… is looking for a couple more primary care practices to test AnticiPal. (in GGC, Lothian, Highland, Forth Valley). Give us a shout if this might be you. #palliativecare

No tweeting from me (@drpoco) yesterday due to a busy clinical day. A significant part of my role as a GP is offering patients and their loved ones the opportunity to plan for their future care. We call this Advance Care Planning (ACP).

Continuing my (@drpoco) PELiCam takeover, let's talk about primary palliative care... Did you know that most people in the UK die at home or in a care home? GPs provide much of this care, along with Community Nurses.

This is a really clear and concise explanation of the fundamentally different roles of primary and secondary care. Massively important in palliative care because of the skill primary care teams have in tolerating and managing uncertainty.

More on @mariecurieuk’s campaign to let dying people access their State Pension - and prevent 1 in 4 dying in poverty - here. This really isn’t something that should have to be asked for - we urge the government to listen to campaigners’ voices and act.

That all said, there are many other factors at play and massive issues around loneliness, depression/low mood, poverty, anxiety and perception of available help. Dying, like living, is unequal and magnifies that inequality in multiple cruel and demeaning ways. 6/6

There is a massive, and difficult, tension between "getting on with life" as best as possible while being aware that death and dying is present in your life. Most seem to manage this through minimising the amount of time thinking about death and dying while not denying it. 5/6
