The statement that 'there is no danger in leaving depression untreated during pregnancy' is misleading. Severe depression can lead to suicide risk, poor self-care, preterm birth and lasting effects on both mother and baby. Antidepressants do have risks but for some women they are what keep them stable and functional. The use of antidepressants in pregnancy comes down to careful and case-by-case clinical judgment.
Prof. Peter C Gøtzsche@PGtzsche1

There is no serious danger of leaving depression in pregnant women untreated with antidepressants. Absolutely none. The clinical effect is far below what is clinically relevant and the drugs double suicides and have many other serious harms. It is naive to think that clinicians can weigh benefits and harms. If they did, no one would use antidepressants.

There is no evidence to suggest medication interferes with the therapeutic process - quite the opposite. One person’s own experience does not outweigh outcome studies.

🧵People are often told the pairing of antidepressants with therapy is optimal, suggesting the dulling of distressing symptoms will create space for deeper emotional work. 1/

@wi_john So much of the cost of these drugs is tied up in the administration of infusions and safety monitoring. Even if they were provided for free, it would be difficult to argue for cost-effectiveness within the NHS.

@ProfRobHoward Has anyone modelled - based on available data - the impact of the shift in NICE thresholds on whether or not these drugs now cross the “cost effectiveness” threshold?

The fundamental problem for lecanemab and donanemab is that it is almost an exaggeration to describe their benefits over 18 months as “modest” and from what little the companies have chosen to tell us about long-term extension data it looks like they do not modify disease course.
john williams@wi_john

Politics vs Evidence? “The independent appeal panel has ruled that some elements the committee used to inform its decision-making in relation to the cost-effectiveness of donanemab & lecanemab need further consideration.” thetimes.com/article/27f32f…

They should be properly tested in careful research studies though. Witness psychedelics. History of psychiatry has been marred by reckless enthusiasm for useless and harmful treatments like insulin coma therapy & leucotomy. Read Eliot Valenstein’s book “Great and Desperate Cures”

Just about everyone knows someone with a mental health condition who is not getting completely better with standard mental health care. New and innovative treatments should be embraced and encouraged. Millions of people are waiting and hoping.

If you actually worked in mental health, as a psychologist or psychiatrist, you’d know that delusions are not “different realities.” They are fixed, false beliefs held despite clear contradictory evidence and lack of logical support. This isn’t a philosophical debate, it’s a well-defined clinical concept. As a philosopher this isn’t your area of expertise, it’s important to approach it with caution and avoid presenting inaccurate interpretations as facts.
Justin Garson@justin_garson

I’m starting to question the very concept of ‘delusions’: the very act of labeling you as delusional implies a superiority on my part - that my grasp of reality is superior to yours in a way that does not merit argumentation. Why would that ever be an appropriate stance to adopt?
