Ontario’s Primary Care Action Plan has beaten its 2025-2026 attachment targets, connecting more than 330,000 people to primary care since it was launched last year.
We’re building on this success with a new Primary Care Medical Record system that will make health care more connected, secure and convenient for patients across Ontario. We’re also launching 124 new primary care teams provincewide and increasing overall funding for the plan to a historic $3.4 billion.
