1. It gives you instantaneous feedback on whether you have the correct diagnosis. There are other opioid mimics that may respond with a minor change in RR with smaller doses, leading you astray with whether further workup is needed. 2. It does not tie up staff on a 1:1 like small dosing does in someone that would otherwise require intubation prior to the invention of naloxone. 3. It allows the patient to participate in their care decisions, especially as many of them did not want to be transported in the first place. An awake patient can accurately receive immediate information about drug abuse treatment options.

When it comes to naloxone opioid reversal, there are 2 schools of thought - give them just enough to increase their RR or totally reverse and wake them up. I'm in the latter. I rarely start with <4mg. There's a theoretical risk that their opioid could outlast the naloxone, they wake up, refuse transport or leave the ED, and then drop dead. That's another myth. In a study of 542 patients completely reversed by EMS followed by refusal of transport, there were 0 deaths within 48 hours.
