Today’s procedure provided a compelling demonstration of meticulous anatomical dissection along avascular tissue planes. The patient presented with chronic cough and neck discomfort, initially suggestive of a prominent right-sided goiter. Further evaluation revealed a large posterior retrosternal extension descending to the level of the aortic arch (classified as BII), which displaced the thyroid cartilage and trachea to the left, causing significant compression. The total thyroidectomy reinforced fundamental surgical principles of precise technique, bloodless dissection along anatomical tissue planes, and unwavering commitment to patient safety. The operation proceeded flawlessly under the assistance of our fellow, Dr. Mohammad Alif Bin Yunus. Our preoperative planning and stepwise discussion proved invaluable, offering a profound opportunity for surgical refinement and lessons learned. We extend our sincere gratitude to our exceptional anaesthetist, Dr. Peggy Cheang, and the entire dedicated operating theatre team at UCSI Hospital for their seamless collaboration and outstanding support.
