How to put eye drops the right way: Eye drops work only if they reach the eye. Many people miss the eye, use too much, or let the medicine drain away too fast. This does not mean they are careless. No one is taught how to use eye drops properly. Good technique helps the medicine work better and reduces side effects. The steps are simple and do not take much time. Start with clean hands Wash your hands with soap and water. Dry them. Check the bottle label. Make sure you are using the correct drop and the correct eye. If the bottle was kept in the fridge, wait a minute before use. Cold drops can make the eye water. Get into a comfortable position Sit in a chair or lie down. Choose a place with good light. Tilt your head back. Look up. Use one finger to pull the lower eyelid down (NEVERE THE UPPER EYELID). This creates a small pocket. The drop should go into this pocket, not onto the eyeball. Hold the bottle above the eye. Do not let the tip touch the eye, eyelids or lashes. This keeps the bottle clean. Use one drop only Press the bottle until one drop falls into the pocket. One drop is enough. The eye cannot hold more than one drop at a time. If the drop misses the eye, try again. Do not rush. Close your eye after the drop goes in. Do not squeeze the eyelids. Retain the drop in the eye Place a finger on the inner corner of the eye, near the nose. Hold it there for one minute. This blocks the tear duct. It keeps the medicine in the eye and limits how much enters the body. This step matters for glaucoma drops and for children. If you use more than one drop Wait at least five minutes before using the next drop. If you use drops too close together, the second one washes out the first. If one medicine is a gel or ointment, use it last unless your doctor advises otherwise. Common problems to avoid Do not blink many times after putting in a drop. Blinking pushes the medicine out. Do not touch the bottle tip to the eye or fingers. This can contaminate the drops. Do not stop the drops because of brief stinging. Some drops sting for a few seconds. Tell your doctor if pain, redness, or blurred vision lasts. One drop, placed the right way, does its job. Image from google

My daily advice to UGs in ward: A viva isn’t about saying everything you know. It’s about guiding the examiner. Speak only what you can defend, don’t overshare, and structure your answers. If you don’t control the flow, the questions turn random and costly. What's your viva hack?

Elderly care in hospitals is far more complex than recording blood pressure readings and prescribing antihypertensives or antidiabetic drugs. A significant portion of the problem lies in lifestyle and social realities, factors that cannot be corrected by any doctor sitting in an OPD. Medications may be prescribed, but their availability is uncertain. Even in premier institutions, drug shortages are common; the situation in peripheral facilities is much worse. Treatment plans often fail not because of poor intent, but because the system cannot consistently support them. Investigations present another challenge. Many essential tests are not available under one roof, forcing geriatric patients to travel to distant facilities. For an elderly person, this is not a minor inconvenience, it is a major physical and emotional burden. Compounding this is the social neglect many elderly individuals face. Some have been abandoned by their children; others live in the same household but receive little to no support for their health needs. As a result, they are left to manage the healthcare system alone. A single hospital visit for an elderly patient often involves climbing stairs, standing in long queues for registration, waiting for OPD consultation, moving to separate buildings for investigations, and repeating this cycle multiple times. Each step increases the risk of falls, exhaustion, and worsening of pre-existing conditions such as hypertension, dyspnea, lower back pain or knee pain or any radiculopathy. These are not isolated issues, they reflect a systemic gap in how we deliver geriatric care. Until healthcare systems, families, and society collectively acknowledge and address these realities, elderly care will remain fragmented, exhausting, and, for many, inaccessible. #PublicHealth #Geriatric #MedTwitter

Central institutes don't have bonds but their graduates often choose to work there because of institutional support and good pay. Try doing that for rural infra and you will see doctors join willingly. Don't defend 21st century slavery in the name of bonds.

When you can't argue facts, resort to ad hominem attacks. You get an MBBS after 5 years. Anything you elect to do after that is voluntary. Many Engg grads also go get a Master's and PhD in a similar time frame. They can also make a similar argument. Just say that we don't want a bond, I can agree with that. But this whole line of "Let XYZ have a bond as well" is petulant nonsense.

Watched Dhurandhar finally, and now I can see exactly why the Aman ka Tamasha gang has gone berserk. There was a clear attempt.....almost desperate......to humanize the terrorists: the headmaster’s son, the devoted father, the man who feels deeply, who bleeds emotions like any other human being. A carefully curated empathy project. But Aditya Dhar dismantles it with ruthless precision. He allows the audience to grieve with Rehman Dakait. He lets you understand his rage, even root for his revenge. And just when your moral guard is down, he turns the mirror—hard. The same man, moments later, is shown screaming war cries over the dead bodies of 26/11. The despair Indians can barely articulate, juxtaposed against the unfiltered jubilation of men who celebrate the slaughter of innocents. That whiplash is deliberate. And devastating. Yes, they are human beings. And yes, they are evil. There exists a darkness so absolute that Krishna himself tells Arjun: हतो वा प्राप्स्यसि स्वर्गं जित्वा वा भोक्ष्यसे महीम्। तस्मादुत्तिष्ठ कौन्तेय युद्धाय कृतनिश्चयः॥ Some wars are not moral dilemmas. They are moral imperatives. One film has effectively dismantled years of intellectual cabalism that tried to launder terror with nuance and sentimentality. Dhurandhar refuses that indulgence. It shows you empathy.. and then forces you to confront where empathy must end. What makes this even more subversive is how the message is delivered. Dhurandhar gives the Salman/Shah Rukh/Animal audience everything they crave: pulsating music, slick action, clean choreography, a steady camera that doesn’t hide behind chaos. And yet, beneath the gloss, it administers a truth far more potent than any sermon. Unlike The Kashmir Files, which hit like an injection....sharp, necessary, but painful...Dhurandhar works like a mother giving a child bitter medicine: coated with honey, swallowed before resistance kicks in. The entertainer becomes the carrier. The commercial film becomes the conscience. And that, perhaps, is what truly unsettles its critics. #Dhurandhar

Having interacted with many Paksitanis, I can understand why liberals react the way they do to Dhurandhar. Most Pakistanis one meets in Europe, UAE, USA are pleasant and smart - if not in the broader context of friendship, at least in the narrower context of business. They are educated, suave, well travelled. They represent the top layer of Pakistani society - and this is what most Indians would come in contact with. Dhurandhar shows the other end , the bottom of the barrel Lyari gangs. No Indian liberal has met a Rehman Dakait, Kasab or a Maj.Iqbal and lived to tell the tale. Their atomized lived experience doesn't match the documented harsh reality of terrorism and crime. Instead of concluding their mental map is wrong, they conclude the terrain doesn't exist. A fatal error. The second reason is they confuse individual relationships with geopolitical ones. An Indian and a Pakistani can be friends, of course. Even Indo Pak couples aren't as uncommon as one imagines. To believe that this will somehow pave way for friendship between the two nations is naive. To think that the potential friendship would be detailed by movies but not cross border terrorist attacks is stupidity typical of an Indian liberal. India and Pakistan are not just two different nations - they represent two different civilizations and world views. Never the twain shall meet. The best one can hope for at national level is a tense detente and eternal vigilance. Like a divorced couple minding their own business, instead of fighting. Sadly, even that is incorrigible optimism.
