Dear RN, When Charting patient’s refusal of medication 💊, Don’t just say ❌Patient upset/angry, refused meds" ✅better say this; At 15:30pm, This RN approached patient for scheduled medication . Patient was observed tearful, pacing at bedside, and speaking in a raised voice. When asked what was wrong, patient stated, ‘I’m tired of all these medications you people are giving me; they are not helping and they make me feel worse.’ Education was provided regarding the significance of the medication and the potential risks of refusal, including worsening of condition and delayed recovery. Patient stated, ‘I understand what you’re saying, but I still don’t want it. They are not helping me.’ Patient remains alert and oriented to person, place, time, and situation, no change in mentation was observed. Attending physician notified @ xx:xx Plan of care Ongoing. Here is the reason why you need to document like this , assuming something happened to the patient and your chart get audited, the jury will clearly see the intent, effort and that you know what you are doing.
