@YounisJoseph @shycollie So I am asking you again.. If a physician who graduated from a standardized residency abroad passed board certification in the US, applied to a job in the US and was hired following an interview, why would this be unfair? Seems to me that its just more competition.

Why do residencies require standardization and not med school: Because you don’t become a licensed Dr after med school? How do residencies know you’re not an idiot: There are safeguards in place to certify your med education: 1. Ecfmg 2. WDMS 3. Step 1 4. Step 2 5. Application 6. Interview If all that fails to detect a dumbass, they still have to go through residency. Seems robust to me

Out of 963 orthopedics residency match spots, exactly 5 were offered to non-US IMGs; that’s about 0.5%. I demand justice for this great replacement

@shycollie @YounisJoseph Also interesting that that these advocates take no issue with the US medical system stealing the best and the brightest from countries around the world. What happened to health equity? These countries need excellent doctors more than the US does.

@shycollie @YounisJoseph They are pulling the ladders up behind them.. Medical school has never been more expensive, and matching into competitive specialties have never been more difficult. Physician compensation is also declining.. Sounds about right to make it even more difficult for medical students.

@YounisJoseph @shycollie Why not create a regulatory body that evaluates residency programs globally, and those who meet the standard should be allowed to practice in the US without US residency training?

@shycollie @arghhhhhhhhh92 You don’t need to complete residency to apply, you need USMLE 1/2 scores and a medical school diploma (with ECFMG certification from a school listed in WDMS). Your question is easily answered by reiterating: every American license must follow American procedures

@YounisJoseph @shycollie Why do residency training and board certification require standardization, but medical school does not? Seems rather hypocritical to me..

@YounisJoseph @shycollie This exact argument can be used in the context of residency applications. How can US residencies verify that non LCME regulated medical schools are up to standard and have fair evaluations of didactic and clerkship performances?

@YounisJoseph @shycollie But lets test your consistency.. Should we also remove the US residency requirement to practice medicine in the US? Why should we not welcome more competition on the attending level if competition is inherently good?

@YounisJoseph @shycollie I think you are missing the point.. Those are taxpayer funded training spots and should go to people who are from and trained in the US. Nobody takes issue with IMG applicants soaping into unfilled spots.

@BrennanSpiegel Versus me badging in and out all day across multiple terminals, logging in and out at each one each time. I probably lose 30 min a day doing that

First day using AI-powered smart glasses in clinic. Real-time EHR. No turning to the screen. Just eye contact and conversation. All the data I need, when I need it, dynamically served up and projected into the room. Even differential diagnosis! Early… but unbelievably good 👇

@BrennanSpiegel I would much rather talk to a physician sitting infront of a computer that I can maintain eye contact with than a person with ridiculous looking sunglasses.. This feels remarkably antisocial and demonstrates everything that is wrong with modern medicine.

@MaryBowdenMD The SAVE trial had a mean adherence of 3.3 hours per night for the CPAP group. The group is thus either sleeping considerably less than what is recommended, or alternatively not adhering to their CPAP therapy. CPAP is first and foremost for symptom relief...

Two biggest studies on CPAP for sleep apnea completely disagree on mortality: SAVE trial: No benefit - CPAP didn’t reduce CV events or death in patients with OSA + heart disease. Lancet meta-analysis: CPAP linked to 37% lower all-cause mortality & 55% lower CV mortality. Guess which one was funded by @Resmed (leading manufacturer of CPAP machines)?

@kneesantos somewhat agree but realistically he wasn't damaging his patients' care, also Santos still needs to be a doctor first and this feud has gotten in the way of her work already several times this season

@alexmd2 @DrDiGiorgio You're easily looking at 6-12 months to see a subspecialist in most areas of the US. Even primary care schedules out 4-8 months now. If you are fine with seeing a nurse practitioner you can get an appointment within a few weeks.
