Published in JAMA with #CCRdownunder: In critically ill adults with suspected sepsis, antibiotic regimens were safely reduced when guided by measurement of procalcitonin but not by C-reactive protein. @CritCareReviews ja.ma/41t10TE

@James_West_PhD @charlot_summers @PLOSGPH That's terrible and very detrimental to ECR. I'm assoc editor and I similarly get passed on papers that have been in the system for ages with little/no progress. Makes it v difficult if the manuscript needs a lot of work to be ready for publication! Agree, its broken.

I’m one of the academic editors at PLoS One, and I have seen enough of this that I know what likely happened. They are having trouble recruiting and keeping academic editors. To their credit, they have fairly high standards- they mostly want people fairly advanced in their careers. However, it doesn’t come with nearly the prestige of being an editor at a society journal, and so they have trouble getting and retaining people. Nearly 1/3 of the papers I handle these days are transfers from an editor who’s no longer available. I’m thinking of quitting myself, after 10 years, because it’s gotten progressively harder to get reviewers, and the lack of editors means I increasingly handle papers well outside my field. What makes all of this worse is that there are some countries that we receive a lot of papers from, but which I never have any luck recruiting reviewers from. I know we all complain about the breakdown of the current journal system from our end, but I feel like it’s kind of breaking down from the other end as well. Reviewing papers is a community service which people increasingly don’t feel called upon to do.

Update: Today @PLOSGPH sent our manuscript back (after 4.5 months) due to their inability to secure an editor to handle the manuscript or any reviewers. This is despite having previously sent us emails saying the manuscript was under review and a decision would be made soon. 1/2

We submitted a manuscript on 17 Jul. 4 months later, only message from journal was to say the original editor now unavailable & they are seeking a new one. We’ve chased twice & been told manuscript remains “under review”. At what point do we withdraw & submit elsewhere?

Presented at #CCCF2024: In the BALANCE randomized clinical trial, patients with bacteremia were assigned to 7 or 14 days of appropriate antimicrobial therapy, and 7 days of therapy was found to be noninferior to 14 days. Full trial results: nej.md/3Z5bTJD

This is what I gleaned the working group recognised and will seek to address (1) the infeasibility of training in academia and two medical specialities (2) academia now linked to pay penalties through slower progression and diminished merit awards….

I’m at an NIHR Senior Investigators meeting where Patrick Chinnery presented on an academic clinical careers and a forthcoming report. Looks v promising from his short presentation.

7 days in the week. 5 digits on your hand. So naturally antibiotic courses are multiples of 7 or 5 🤔. @SHORTERtrial large @NIHRresearch funded trial investigating short (5 day) course of antibiotics vs SoC in sepsis. Challenging the 'week's course' dogma.

New data released today in @TheLancet shows that without urgent action, antimicrobial resistance could kill 39 million people by 2050. Deaths and inequities from the global antibiotic emergency are increasing and now is the time to take urgent action. thelancet.com/journals/lance…

There is a crisis in clinical academia in the UK. We talk of the need to expand medical schools, embed research into the NHS to improve care, & the importance of a vibrant life sciences sector for the economy of the UK, & yet we seem unable to tackle the underlying issues 1/9
