Wow, I feel like every pharmacy just experienced this same problem during the DIR hangover cliff that happened the start of 2024. How many more will close down if they can’t randomly pull tens of thousands of dollars out of their ass for 2026? Thousands of pharmacies will be gone 1st quarter of next year. Big applause 👏 to all of the people who made this possible due to the Medicare negotiated drugs. FIRE THE PHARMACY BENEFIT MANAGERS? No, let’s just add another layer to the already over-cooked cake.

This is why our system is crazy. Brand drug manufacturers subsidize hospitals w 340b and PBMs many ways, and the insurance companies that own them Hospitals/providers subsidize insurance comps, which also take advantage of CMS/HHS/taxpayers and employers

@joeymattingly Medical billing is the same. Common practice is to charge 1.5- 2.5x Medicare to ensure the reimbursements are captured when changes are made. Trust me. They make their contractual adjustments. The whole system is stupid & needs reform.

This is a very helpful reply. Pharmacists are complicit in “overbilling” on the chance they will get overpaid - to compensate for all the situations where they could be underpaid. This is an awful system. We should change it…
