#Finerenone in #CKD without #DM : FIND-CKD Trial 📍Finerenone slowed CKD progression in non-diabetic CKD, reducing annual #eGFR decline vs placebo (−3.3 vs −4.0 mL/min/1.73m²/yr; P<0.001). 📍Finerenone ⬇️ kidney/CV events by 23% (HR 0.77, P=0.04), supporting cardiorenal protection beyond eGFR preservation. 📍HyperK+ was more common with finerenone (17.0% vs 13.3%), but treatment discontinuation remained low (1.5%). nejm.org/doi/full/10.10… @NEJM

💊 Practical algorithm for diuretic resistance in acute HF from #HeartFailure26— this is the slide to save! Start: Furosemide 120-160 mg ↓ Still congested? Add Chlortalidone 12.5-50 mg/24h ↓ Still congested? Tailor to the problem: • Metabolic alkalosis → Acetazolamide 120-500 mg/24h • Hyponatremia → Tolvaptan 15-50 mg/24h • rLVEF + hypopotassemia → MRA 25-100 mg/24h And running through the whole algorithm: ➕ Add iSGLT2 throughout ➕ Add sacubitril/valsartan if LVEF <49% ➕ Consider parenteral route if oral absorption is the issue Clean.
