In voluntary euthanasia, which has growing international acceptance, clinicians fulfill competent patients’ voluntary requests to relieve their suffering by intentionally ending their lives. In Canada, voluntary euthanasia was legalized in 2016 after the Carter v. Canada Supreme Court decision and now accounts for roughly 4.7% of all deaths. (Canada’s eligibility criteria are listed in Box 1; key milestones leading to current legislation are listed in Table 2.) Legalization of voluntary euthanasia introduced a new form of controlled death to which organ-donation regulation had to adapt. The hypothalamus is a small brain region whose primary function is to maintain a stable internal environment, as first described by the French physiologist Claude Bernard and later by Walter Cannon, who coined the term “homeostasis.” In 1901, Austrian physician Alfred Fröhlich described a case report of a boy with rapid-onset severe obesity and hypogonadism due to a tumor near the pituitary gland. Subsequently, Anand and Brobeck found that disruption of the hypothalamus (rather than the pituitary) causes weight gain, establishing the critical importance of hypothalamic pathways in weight regulation. Within the hypothalamus, distinct clusters of neurons, or nuclei, form a complex network of circuits that receive input from, and project to, other brain regions. Hetherington and Ranson found that lesions of the ventromedial nucleus and arcuate nucleus cause hyperphagia and obesity in animals, whereas disruption of the lateral hypothalamus causes the opposite — hypophagia and leanness. These findings indicated that the precise location and extent of hypothalamic damage determine the effect on body weight (shown in illustration). Case Records of the MGH: A 68-year-old man was evaluated in an outpatient clinic because of fatigue and dyspnea. The patient had a history of COPD with progressive bronchitis symptoms that had resulted in frequent emergency department visits and hospital admissions, as well as the use of supplemental oxygen during exertion. Three years before the current presentation, bronchoscopic lung-volume reduction surgery was performed, with placement of four endobronchial valves. One year later, symptoms progressed.
