Are you a lumper or a splitter? This question keeps dividing colleagues (and friends!) working on psoriatic arthritis and axial spondyloarthritis. At conferences, in papers, over coffee, the debate never ends! While these conditions overlap clinically, recent evidence suggests fundamental differences in their pathophysiology. AxSpA may be characterised by predominant bone involvement with perifibrocartilaginous SIJ osteitis, whereas axPsA may manifest primarily as inflammation in ligamentous soft tissue or "ligamentitis". The problem? We can't routinely biopsy the sacroiliac joint to explore these differences at a tissue level. So we asked: could radiomics serve as a "digital biopsy"? In this proof-of-concept study, we extracted 120 radiomic features from SIJ bone marrow oedema on MRI. Our XGBoost classifier achieved 80% accuracy in discriminating axSpA from axPsA using texture, shape, and intensity features alone. No clinical parameters needed! We also found that shape-based features like sphericity and elongation were associated with TNFi persistence in axSpA patients. I should be transparent about the limitations. This is a single-centre study with 66 patients, and we haven't validated these findings externally. The survival analysis was exploratory with only 10 discontinuation events. These results need confirmation in larger multicentre cohorts before clinical application. That said, the single-centre design also means all MRIs came from the same scanner with standardised protocols, which minimises the noise and artifacts that typically arise from heterogeneous imaging equipment, giving us cleaner radiomic signals to work with. If validated, radiomics could offer a non-invasive window into what makes these conditions potentially distinct. So, what team are you on?
