Regardless, too complicated to discuss on X. Happy to grab coffee at STS & get others thoughts as many options on how to handle this one

This is a perfect example of why we need to reconceptualize “unresectable”. 3 components of resectability: 1) technical 2) patient physiology 3) oncologic Each of these is fluid & may change throughout a patient’s Tx course Just because Brian’s patient at outset was technically unresectable, should not = never surgery. In the IO era, we will be resecting more residual disease in patients who appeared borderline or unresectable upfront.

Pancoast tumor invading 1st rib and no plane between mass and subclavian artery. Good operative candidate. Induction Chemo-XRT? Or Induction Chemo-immuno?

For non-pancoast I agree- often get great results with chemoIO even if low PDL1. Difference is guidelines support it. Hard to go off guideline for pancoast (chemoRT) unless there’s a compelling reason. But every patient is unique…

High pdl1, so after tumorboard, gave chemoIO. Became a fairly straight forward RUL, albeit sticky. Complete path response. Can’t change practice guidelines on single anecdotes tho.

Looking for opinions, following my dear colleagues examples: Young woman, carrier of a MEN1 germline mutation, with multiple pancreatic NETs (insulinoma). History of partial pancreatectomy (details unclear). Now symptomatic, with tumors in the head, uncinate process, neck, body, and tail. No distant mets. She is strongly against a total pancreatectomy.
