Ground beef is made from scraps and trimmings. The big hunk of white is beef fat added to the processor to achieve a specific muscle-to-fat ratio, such as 80/20 (80% muscle to 20% fat), 90/10, etc. It appears to be a large piece of fat that was not ground up correctly. This is probably not a health issue or any other risk; it is just a quality control mishap.
