The conclusions of this study FAR transcend anticoagulation decisions. Yes, don't switch VKA->NOAC in frail pts. But the more important message involves evidence-based practice. The authors powered the study for NOAC superiority. B/c that is what the trials found in younger healthier pts. But. Boom. They ended up terminating the trial early b/c of futility and harm The message is that we need to be super careful and humble about assuming evidence in younger healthy pts applies to older frailer pts with multi-morbid conditions #esccongress #EBM @AndrewFoy82
