Dependence and addiction have entered the chat incorrectly again. So let’s make this painfully simple: If you drink coffee every day and suddenly stop, what happens? Headache. Fatigue. Irritability. That’s dependence. Your body adapted, and now it’s reacting. That is not addiction. Addiction would be you breaking into a Starbucks at 6am, hopping the counter, and risking a felony for a caramel macchiato. That’s the difference. Dependence = your body adjusting. Addiction = compulsive behavior despite harm. Now let’s talk about dependance withdrawal, because people keep pretending it’s not real. It is. I’ve experienced it. March 2nd, 2026, I had my prescription for oxycodone IR in hand and still couldn’t get it filled. Pharmacy after pharmacy turned me away. By the end of the day, my body started reacting. Vomiting. Goosebumps. That full-body, skin-crawling “something is wrong” feeling. And my pain? Off the charts, worse than my baseline. Again…That’s dependence withdrawal. And before anyone says it, yes, the physical symptoms of withdrawal can look very similar whether someone is dependent or dealing with addiction. The difference isn’t the symptoms. The difference is why they’re happening. Not because I misused anything. Not because I’m “addicted.” Because my body is physically dependent on a medication I take exactly as prescribed. Pain coming back after stopping a medication is real. Withdrawal is also very real. Sometimes one overshadows the other, but that doesn’t mean it isn’t there. Blurring this line, intentionally or not, is how patients get mislabeled, undertreated, and dismissed. And we’re way past the point of pretending this confusion is harmless.

This MD proudly displays a sign stating “No opioids prescribed” in his clinic. Yet in his video rating pain txs, he gives opioids a 10/10-calls them “an excellent tx option for some pts”. Opioids are effective, but he refuses to Rx them. Who absorbs the decision? The patients.

Withdrawal alone isn’t proof a substance should be illegal. If it were, we’d have to ban caffeine. I drink espresso daily & if I stop, I experience very real withdrawl symptoms. Most people recognize that as normal caffeine dependence, not a reason to ban coffee. #kratom

If you can watch someone suffer in intractable pain and justify it by calling them “drug seeking,” you’ve lost your humanity. Period.

@dsowens17 @hoss_tagge Substances don’t cause addiction, they may cause dependence but addiction is far more complex than the substance. Conflating the two not only harms patients with chronic pain, it also harms addicts.

@hoss_tagge Opioids are highly addictive even when taken as prescribed. Many people who were heavy smokers never developed lung cancer. This is not evidence that smoking is safe.

No one dies from taking rx opioid medication as directed. Millions of patients have used these medications for >10 years to good effect. More patients die from NSAIDs than from rx opioid pain meds. Stop misinformation propaganda & hysteria. Sign the No Opioid Pledge.
