كان أخي رحمه الله يعمل في مركز الأمير سلطان لأمراض وجراحة القلب لمدة ليست بالقصيرة شارك خلالها بعشرات ان لم تكن مئات من عمليات القلب خاصةً للأطفال وسافر إلى اليمن والمغرب لإجراء عمليات قلب لأطفال ضمن برامج خيرية انسانية ساهم أبا دعد بانقاذ أرواح عديدة، وها هو يودعنا بعمر ال 46 عاماً 💔 تاركاً إرثاً من الذكر الطيب والعمل الخيّر وذرية صالحة بإذن الله اللهم اربط على قلب والدتي ووالدي وقلوبنا جميعاً واجعلنا ممن اذا اصابتهم مصيبة قالوا انا لله وإنا إليه راجعون أعوذ بالله من الشيطان الرجيم أعوذ بالله من الشيطان الرجيم أعوذ بالله من الشيطان الرجيم مصابٌ جلل الحمد لله الحمد الحمد لله لله ما أخذ ولله ما أعطى

Tricuspid Valve Memory Hooks 1️⃣ S = S Septal leaflet = Septum Always attached to the interventricular septum If it touches LV → it’s septal, no thinking needed. 2️⃣ C = P Coronary Sinus = Posterior leaflet See the coronary sinus in apical 4-ch? 👉 The leaflet next to it is posterior. 3️⃣ O = A Outflow (RVOT) = Anterior leaflet RVOT or pulmonary valve in view? 👉 That leaflet is anterior. 4️⃣ Big = Anterior Largest, most mobile tricuspid leaflet = Anterior If one leaflet dominates the image → anterior. 5️⃣ 4-Ch = S + P Apical 4-ch mainly shows: Septal (medial) Posterior (inferior, near CS). 6️⃣ 5-Ch = A appears Tilt up → aorta comes in When LVOT appears, anterior leaflet shows better. 7️⃣ Moderator Band = RV side Find moderator band → confirms RV Leaflet on that free-wall side = Anterior. 🧠 Rapid Memory Hooks ● Septal leaflet → attached to interventricular septum. ● Posterior leaflet → next to coronary sinus. ● Anterior leaflet → faces RV free wall / RVOT and is largest.

Mitral Valve: Structure & Function The mitral valve is a bicuspid (two-leaflet) valve that regulates blood flow between the left atrium & left ventricle, ensuring one-way circulation and preventing backflow. It consists of: ✔️ Leaflets (Anterior & Posterior) ✔️ Annulus (fibrous support ring) ✔️ Chordae tendineae (tether leaflets to muscles) ✔️ Papillary muscles (Medial & Lateral, preventing prolapse) 🔹 Anterior leaflet = longer, but attaches to a smaller annular portion 🔹 Posterior leaflet = shorter, but divided into three scallops (Medial-M, Central-C, Lateral-L) 🔹 Both leaflets attach to both papillary muscles, ensuring efficient closure & preventing regurgitation A vital structure in maintaining proper circulation.

@Murmur_MD Same but I add additional safety wire across (usually Safari wire) in case need to go and do more plugging or do AV rail- all depends on how easy to access the hole and if Maximum support needed.

PVL cases are always an adventure! A few tips for #Toolkit Friday from a smooth posterior PVL: • Posterior transseptal sets you up for success • Agilis → MP + glide makes crossing predictable • Swap to Amplatzer ES + 0.018 safety wire early for control • TV2 sheath through the Agilis = excellent trackability (I’ve found it smoother than a shuttle) • AVP II sizing: used a 10 mm with “meat” left on the atrial side so the disc faces the ventricular portion Curious what others are doing: – Anyone using a 2.5 mm VBX for the tootsie-roll technique? – Other tricks for tough or crescentic leaks? #CardioTwitter

JUST OUT! #JACCINT @JACCJournals on long-term outcomes of SAPIEN3 MVIV MVIR VIMAC #TMVR. As expected, MVIV did much better than MVIR and VIMAC in all-cause and CV mortality. Median follow-up 3ys (up to 8y) with ~7% severe SVD after 5y and ~10% TMVR failure after 4y. Depends on how you see it, I see this as sobering outcomes: MVIR and VIMAC should be offered only in inoperable patients with limited life expectancies, given poor survival and limited valve durability. MVIV has better outcomes, but need to follow re: SVD and TMVR failure and may end up being a bridge to reop MVR especially in younger patients. 5y durability data of native TMVR are emerging (Intrepid, Tendyne), will see how they fare vs balloon-expandable TMVR in failed valves, rings, MAC. Early experience of on-demand ICE usage in TTVR with Evoque. Our first 100-commercial patient experience now published in #EHJ #EHJVSHD On-demand ICE needed more in patients with CIED with no difference in outcomes. Procedure length improved over time. academic.oup.com/ehjvshd/advanc…

كولي وديباكي عاشا في عصر ذهبي من الشغف المستمر، آلاف العمليات دون رقيب أو حسيب أو قوانين طبية و"Quality control" والورق الممل الذي يجب أن تملأه قبل التقدم لخطوة واحدة للأمام. وصلوا مرحلة التحدي على تشخيص الحالة وفي مرة كان على المخطيء أن "يأكل" قطعة من جدار الشريان المتوسع وحين ثبت خطأ كولي بكل بساطة طلب بوضع القطعة في الكحول لحفظها وطلب طاقم تصوير وأكل القطعة، واستمرت حياة التحدي. لا يوجد عملية في جراحة الأوعية والقلب لا يطلب فيها Cooley clamp or DeBakey clamp.
