In Urology, I saw men who were sad that they have to come change suprapubic catheters always. I saw men who were sad because we had to put hands and biopsy needles in their anus to collect tissue. I saw men who looked at their children with sadness because they got their diagnosis of prostate cancer. I saw men who had to go around with urine bags because the couldn’t control the sphincters no more. I saw men who dropped tears because we had to pad their boxers with tissue to absorb the blood that comes out after the biopsies. I saw daughters who constantly begged to stay away and not see their father’s nakedness. Some even begging us to help dress the man up. I’m surprised that despite all these, someone else saw differently.

Any facility that runs with just a doctor and a nurse is not a standard hospital. I guess there might be a name for such places. Healthcare is team-based for a reason. There are established SOPs and regulatory requirements that guide how health facilities should operate. As basic as it may seem, drug dispensing without a pharmacist increases the risk of medication errors. A functioning system also needs a laboratory scientist for accurate diagnosis and proper documentation. Down to the non-medical staff, they are part of the system that keeps a hospital running. So let’s call things what they are. That setup is not a hospital, it’s just a limited care facility.

Let's stop giving Omeprazole for months to same patients. After 1 month of proper empirical treatment (H.pylori and co) + lifestyle mod., and they don't get better, send to a gastroenterologist. Some have gastric cancer undetected on time.

@Migeloune @ajimotokanoluw3 once someone doesn’t respond to ppi within a month , the proper thing is to have a scope . You shouldn’t actually continue treating blindly without confirming. the extended cause is for proven gerd
