This is a legitimate concern. It is a real risk. I might not have enough diesel to drive to Castlebar for my clinic next week. I take it for granted that I can stop in Headford on the way to refuel. There is no Dart connection between Galway and Mayo, for information. The Galway Limerick trains are different from the trains to Dublin, again just for information. Back to the topic: Also, if I do make it to Castlebar next week and think someone should have chemotherapy, they will have to wait at least 4 more weeks to start. They won’t have AHOS support to avoid ED attendances. They won’t have a clinical nurse specialist to call if they are unwell at home. I mentioned 4 weeks. Sometimes 8 weeks. That is the spread. Sometimes with treatments that are less likely to be effective than treatments that are currently available (manufactured in Ireland, some of them, again for information). Sometimes staying on chemotherapy without measuring its effectiveness because CT scanners are out of capacity. Sometimes, I will decipher handwriting on paper charts, call other hospitals for lab results, tests that I ordered, to find out a result that cannot be shared with me because “GDPR”. Sometimes sitting in an empty clinic building, without as much as a prescription pad. Sometimes we are promised new infrastructure for cancer services across the west. Plans are afoot, boards have been appointed. Sometime, these problems will be addressed. I cannot understand why public healthcare expenditure in Ireland is so high, and in practice the end product is at best rough around the edges. Earlier today, the @HSELive approved the reimbursement of dostarlimab with chemotherapy for the first line treatment of advanced endometrial cancer that is MisMatchRepair protein deficient (dMMR). Thank you to everyone at the HSE who was involved in making this decision. It has been some time since standard-of-care treatment for advanced endometrial cancer was available publicly in Ireland, outside of free drug supply schemes from pharmaceutical companies. As of the 1/4/26, I can retire my automatic first question of "Do you have insurance" in these cases, and instead check the pathology report first. Thank you @hseNCCP, @Tony_CanavanHSE, @CcoHse, @HSELive , @INFO_NCPE and specifically the HSE Drugs Group, who make some of the toughest decision in public expenditure that are to be made these days.
