Another study from our JUPITER group 👇 NSTEMI in the “oldest-old” (≥85) — time to rethink the reflex Real-world data (n=967) shows: 🔺 Mortality is much higher in ≥85 vs younger patients 🔺 But revascularization (PCI) cuts mortality significantly (HR 0.56) In ≥85: 🟢 PCI: 37.9% mortality 🔴 Conservative: 63.6% mortality Yes—complications are higher. But age alone isn’t the story. 👉 Diabetes, renal failure, anemia, low albumin > chronological age Bottom line: Age ≠ contraindication. Select wisely, treat actively. This study may provide clinically relevant evidence to inform the management of patients with mitral bioprosthetic valve failure. SURVIV is an investigator-initiated, prospective, multicentre, open-label, randomized controlled trial enrolling 150 patients with failed mitral bioprostheses eligible for either redo surgery or transcatheter valve-in-valve intervention. The primary endpoint is the composite of all-cause mortality or disabling stroke at 12 months. To maintain a relatively small sample size—given the challenges of randomizing this population—the trial is powered based on a large expected treatment effect.
