When we applied strict RCT inclusion criteria from ARREST, PRAGUE, and INCEPTION to a real-world ECPR cohort published in Resuscitation Plus, something striking emerged: survival wasn’t confined to those who “fit the rules.” Some patients who would have been excluded by trial criteria survived — even with meaningful neurological recovery. RCT frameworks guide us, but at the bedside, rigid thresholds may cost lives.
