MN DHS has an internal audits tracker focused on remedying fraud, waste, and abuse - this includes Medicaid programs and contracts/grants. It is an Excel spreadsheet that is compiled by each admin area and Internal Audits division. Some of the audit findings go back to 2014 and the same failure points that allowed fraud to occur TODAY were identified over a decade ago. Why is MN DHS still facing the same failures that were seen more than 10 years ago without any resolution in sight? Legislators should ask: What happened to audit tracking systems meant to prevent repeat failures? Why were known issues not fully addressed? What guardrails existed when Housing Stabilization launched? Minnesotans deserve answers: Who knew what—and when? What actions were taken? What warnings were ignored? This isn’t about blame—it’s about accountability. Call leadership to testify. Follow the structure. Follow the decisions. Follow the money.
