An excellent overview of #SMOULDERINGMS - how we see progression independent of relapses #PIRA, what processes are involved and what #biomarkers will let us ultimately lead to individualized treatment!

#BrainAtrophy and #CognitiveImpairment in #MS - Closely interlinked, and of great importance to PWMS. Cognitive impairment being associated with more care needs and worse quality of life - focus on slowing/stopping this is essential!

@cathyem1 Great question! It's very hard from a single study to be sure if it's
atrophy->CVR, or
CVR->atrophy, or
<->
There is a lot of evidence of CVR impacting brain function long term generally tho! More work is ongoing and we think simva might have a direct role in PMS protection!
English

@MsStat2 Interesting work. For the lay person (me), does this indicate that reduced brain volume as part of SPMS contributes to earlier than ‘normal’ heart and circulatory disease? Therefore high dose statins would potentially be beneficial for those with SPMS diagnosis?

@aid4disabled @KlausSchmierer @QSMSCENTRE @QMULBartsTheLon @pctuqmul @SvetlanaMilca @AndreaStennett @Kimberley_AP @GavinGiovannoni @monicacmarta @mssocietyuk @Barts_Charity STAT2 has EDSS primary with many others, including 9HPT, 25FW, and the detailed BICAMS cognitive assessments as secondary outcomes.
OCTOPUS slightly complicated as multi-phase, but the phase 3 element likely to use a composite primary (EDSS, 9HPT and 25FW), similar to ASCEND.
English

@KlausSchmierer @QSMSCENTRE @QMULBartsTheLon @pctuqmul @SvetlanaMilca @AndreaStennett @Kimberley_AP @GavinGiovannoni @monicacmarta @MsStat2 @mssocietyuk @Barts_Charity Yup pleased London based trials are dong this. What about other trials in the UK?
English

All of this happening in #MS #trials including #ChariotMS #OHAND #AttackMS, and I trust #OCTOPUS @QSMSCENTRE @QMULBartsTheLon @pctuqmul @SvetlanaMilca @AndreaStennett @Kimberley_AP @GavinGiovannoni @monicacmarta @MsStat2 @mssocietyuk @Barts_Charity
Patrick B, I walk very slowly@aid4disabled
Disability is a complex issue in MS and MS trials Why not combine EDSS scores with the timed 25-foot walk (T25FW) and the 9-hole peg test (9HPT). Surely this is fairer and more complete assessment of disability #MS #disability multiplesclerosisnewstoday.com/news-posts/202…

@cathyem1 @Brandon_Beaber High levels of nerve damage detected in blood probably a bad thing whether on simvastatin or placebo. But the statin did preserve brain volume and performance on some thinking tests as well - so looked promising.
English

@Brandon_Beaber @MsStat2 What does this mean for me and others on the trial? Decline is happening regardless of statin/placebo use?
English

Higher serum neurofilament light chain (sNfL) is linked to more cognitive decline in MS. Data from the MS-STAT study (simvastatin vs. placebo in progressive MS) pubmed.ncbi.nlm.nih.gov/35946107/

English

@cathyem1 @Brandon_Beaber We will look at this again in STAT2, but need to wait for all the final visits to be completed first.
English

@cathyem1 @Brandon_Beaber Hi Cathy. This analysis (using the previous MS-STAT1 trial data) showed that those with having higher levels of nerve damage in your blood at the beginning of the trial was associated with some worsening of cognition over the next 2 years.
