🚨 Prostate cancer care has changed fast. Here’s the 2025 snapshot. The new Lancet Seminar lays out how prostate cancer management has transformed across the disease spectrum 🌍🧬 🔍 Early disease Active surveillance is the default for low-risk tumors, avoiding overtreatment while maintaining excellent outcomes. MRI-first pathways improve detection of clinically significant cancer. ⚖️ Localized to high-risk Surgery and modern radiotherapy offer equivalent cancer control. Hypofractionation and SBRT shorten treatment without compromising efficacy. High-risk disease needs multimodality care. 🚀 Metastatic hormone-sensitive ADT alone is no longer enough. Doublet or triplet therapy with ARPIs ± docetaxel is now standard, with prostate RT benefiting low-volume disease. 🎯 mCRPC era Treatment is biology-driven: PARP inhibitors for HRR alterations, Lu-177 PSMA radioligand therapy after ARPI and taxanes, cabazitaxel preferred over ARPI sequencing. 🧠 Big takeaway Sequencing, patient selection, toxicity, and shared decision-making matter as much as drug choice. 📖 Valérie Fonteyne et al. ✍️ Appunti di viaggio Jannik genera un insieme di emozioni sempre positive. Una calamita. Un respiro che ti arriva. Vivi le sue vittorie, stai male se perde. Catalizzatore di sentimenti. Ma soprattutto, come tutte le cose più preziose, va maneggiato e trattato con cura. La vicenda Davis ha scatenato i giudizi peggiori su di lui. Non mi riferisco solo ai social, sempre trattati con diffidenza da Jannik, non ritenendoli la vita reale. Ma ha generato corti circuiti mediatici poco eleganti anche da chi dovrebbe conoscere i fatti prima di esprimere giudizi molto superficiali. Ovviamente Jannik non ha dedicato neppure un secondo della sua vita a chi cercava di creare inutile clamore. Non ti curar di loro, ma guarda e passa. Anche perché Jannik passa e arriva alla gente molto più di chi ne parla in modo sbagliato. Ho avuto la fortuna di seguirlo nei grandi tornei in giro per il mondo e una sensazione forte l'ho sempre avvertita tra le tante interviste e il suo lavoro sul campo d'allenamento. Se sul campo è il modello da seguire per tutti, è un esempio altrettanto fenomenale fuori dal campo: per educazione, rispetto, valori, spessore umano. Non si può non ammirarlo. È il modello ideale per tutte le nuove generazioni. Uno così nasce una volta ogni 50 anni, almeno. È come Enzo Ferrari, un genio che un giorno spiegò come l'invidia aggredisse chi aveva successo. Jannik è la Ferrari. Il made in Italy che tutto il mondo ammira. È top class, n.1 al mondo per 65 settimane, campione del mondo con la Coppa Davis vinta per due anni di fila e porta in giro per il mondo la bandiera dell'Italia 365 giorni l'anno. E lo farà per i prossimi dieci anni. Chi non conosce i fatti o non ha la cultura sportiva non può capire. Molto più italiano e ricco di umanità di loro. È una questione troppo più grande. È la forza dei sentimenti che scatena lo sport. Chi non lo capisce si perde qualcosa di unico. Perché Jannik è un sentimento che nutre l'animo. ✍️ Angelo Mangiante
