Thanks! Refusal triggers on empty retrieval or weak retrieval (reranker's best score falls below a floor), it's a grounding check, so the model never answers confidently without real support/evidence. For eval I use IR metrics instead of promptfoo style LLM judged eval, because if retrieval actually fails, a smart LLM can still write a fluent enough answer to fool an LLM judge into passing it. I hope this is helpful.

The client was a doctor from my hometown who needed a portfolio website. I built it, delivered it, and got paid. I still remember checking my account again and again to make sure the payment was real. That $50 gave me something much more valuable than money. It gave me confidence.
