Distal transradial access for angiography and PCI. Compared with classic transfemoral access, proximal (conventional) transradial access (pTRA) has been associated with fewer access-site-related complications, earlier patient ambulation, and greater postprocedural comfort. In ACS patients, pTRA has also been linked to lower mortality rates. Distal transradial access (dTRA) offers advantages over pTRA, including faster hemostasis and lower rates of proximal radial artery occlusion, the most common complication of pTRA. Despite the increasing adoption of dTRA by interventionalists worldwide, data on routine coronary angiography and/or percutaneous coronary intervention (PCI) via dTRA remain limited. In this study, we aimed to evaluate differences (chronic vs. ACS) using a large-scale, real-world registry of consecutive and all-comers patients.
