Great to be back from Nepal! In this week's first Substack article, we discuss a recent paper written with Chris Kempes on the role of energy in health and disease. The key difference between life and death is not the physical stuff of life. It's the flow of energy. We propose that thinking about health and disease processes energetically can unlock a meaningful transition to the next stage of evolution for biomedicine.

Paper from myself, Dr. Braunwald and others on the use of AI to guide shared decision making. We conduct trials and draw inferences from / about populations. We treat individual patients, however, who have innumerable combinations of subgroups with second and higher order interactions (which can be modeled with ML/AI). We see patients in clinic and iteratively reassess their risk moving forward based upon the presence or absence of events (including multiple events) & their tolerability of a drug / device (eg a patient who has not bled is at a lower risk of bleeding moving forward) We should also take into account individual patient preferences (not population preferences) with respect to ischemic / bleeding outcomes and side effects. Exciting new research reveals that phosphoenolpyruvate (PEP), a natural metabolite produced during glycolysis, acts as a key defender against chronic #inflammation during aging. Scientists discovered that PEP levels rise in the body during early #aging as a protective response, directly inhibiting the cGAS-STING pathway - a central driver of age-related inflammation and disease. This "self-protective" mechanism is evolutionarily conserved in both mice and humans, but as advanced aging progresses, PEP levels decline, leading to exacerbated inflammation and functional decline. The findings offer promising therapeutic potential: supplementing PEP in aged mice successfully reduced inflammation, improved cognitive function, extended healthspan, and even alleviated #Alzheimer's disease pathology. In humans, higher PEP levels in older adults strongly correlated with better kidney and liver function and lower inflammatory markers. This positions PEP as a compelling candidate for interventions aimed at promoting healthy aging and combating age-related diseases. nature.com/articles/s4358…

FDA authorizes resumption of Intellia’s heart‑disease gene therapy trial after a safety pause. Last year, the FDA paused Intellia's two trials of the ​gene-editing treatment after a patient in the heart disease study was ​hospitalized with severe liver complications and later died.
