Today is Lynch Syndrome Awareness Day. Lynch syndrome is the most common hereditary colorectal cancer syndrome, yet it remains underdiagnosed. Up to 1 in 300 individuals may carry a mismatch repair gene mutation, but many are identified only after developing cancer. As surgeons and clinicians, this has real implications: • Early-onset colorectal cancer • Synchronous/metachronous malignancies • Risk beyond the colon: endometrial, ovarian, gastric Universal tumor testing for the mismatch repair deficiency is no longer optional. It’s essential. Identifying Lynch syndrome doesn’t just change one patient’s care; it impacts an entire family. What comparator would be used to empirically demonstrate that patients achieving pCR may safely skip surgery? Would this be based on a synthetic control arm, as accepted by the FDA in selected circumstances, using propensity matching to EVP+ radical cystectomy pCR cohorts (e.g from 303/304)? A critical point that we still see often after EVP pCR during path assessment is the persistence of CIS that seems not to be served by EVP - wonder how this will affect longer term bladder intact survival as it is the root of MIBC and potential relapses.

Two babies died in Samoa when a nurse incorrectly mixed a muscle relaxant into the measles vaccine. The vaccine didn’t kill them. The nurse’s negligence did. That didn’t stop RFK Jr from going to Samoa to spout anti-vaxx rhetoric. Vaccination rates dropped & 80+ kids died.
