Radial artery occlusion remains an important complication of transradial angiography. Preprocedural USG to guide sheath size selection appears to reduce this risk. In reviewing the literature, I came across varying approaches. What're the cutoff values in your daily practice?

Dr G Rajesh (Gopalan Nair Rajesh).
With my cardiology teachers Prof Rajan Joseph Manjuran who passed DM Cardiology from AIIMS in 1974 and Prof N Sudhayakumar who passed out DM from PGI Chandigarh in 1983. These brilliant teachers who dedicated their life for teaching students stimulated me to follow their path.

Octagen needs hernia surgery. Anesthesia refers to cards due to age >>echo EF 25%>>Cath. Note LM and LAD are both heavily calcified. Result below (RCA is normal). Where do we go from here. CARP, REVIVED BCIS2.

If no surgeon available in Mumbai even with emergency transport, then IABP or Impella, IV 2b3a. Next evaluate resolution of symptoms. If still symptoms unrelieved then wire all 4 branches. Next PTCA of LM into LAD, followed by DES LM into LAD. Then evaluate hemodynamics, angiographic appearance. Less is more acutely, if patient stabilizes, STOP and plan transfer to surgical center.
