New paper!! On the evidence base for clinical guidelines on risk assessment tools for people who self-harm. We raise concerns about English 2022 NICE guideline on self-harm, which reached unusually definitive recommendations on risk assessment tools based on 2 studies. /1

Our new paper using focus groups with people in prison and staff. Finds support for the use of a risk tool to bridge the transition from closer supervision back to the normal regime
Seena Fazel@seenafazel

Repeat self-harm in prisons is associated with substantial health morbidities, costs, disruptions to care and rehabilitation, and suicide (in prison and on release). Just out - our qualitative study on using a structured framework (RAPSS) to assess risks of repeat self harm 👇

And the role of medication? Was that considered? Why not? Approximately 16.9% of men and 47.9% of women in prison were found to be on some form of psychotropic medication (which includes antidepressants, antipsychotics, and sedatives)

journalslibrary.nihr.ac.uk/hta/published-… Involved focus groups with people in prison and different staff members. Open access. Collab w/Manchester and York Uni colleagues (who managed to complete it despite various hurdles along the way).

Excellent blog post from The Mental Elf on the high levels of mental and physical health problems among people in prison. Based on our umbrella review, where we synthesized the review evidence on prevalences of medical conditions (incl. infectious diseases) 👇

What's the effect of family income on a child's IQ? Between families, we can see that higher-incomes are associated with kids having much higher IQs. However, the sibling exposed to higher incomes in childhood isn't likely to have a much higher IQ. They're little better off!

New paper! External validation of a risk assessment tool for released prisoners (OxRec) in Norway. Provides evidence-based method to improve linkage with aftercare services (esp. substance misuse, mental health). Sample >26k, AUC 0.77 for violent recidivism, recalibrated model /1

📢 New Article @BMJMentalHealth 👇🏼 Clinical guidelines on self-harm & suicide prevention must reflect uncertainty in the evidence base. A new perspective by Aida Seyedsalehi & @seenafazel critiques NICE recommendations on risk assessment tools & calls for more rigorous impact research. Prediction models shouldn’t be prematurely dismissed or implemented without high-quality evidence. In suicide research, much UK expert opinion has been surprisingly fixed considering uncertainty in evidence. Also different to forensic psych, where tools assist clinical decision-making, also for rare outcomes, and provide consistency within and between services.

In summary, time for a change really. The field has moved on from symptom checklists, sometimes misused (e.g. not engaging) and leading to denial of care. Whether and how risk assessment tools can support and augment clinical decision-making needs more research.

In our 2025 systematic review of performance of suicide risk models, we identified *91* papers describing development of 167 models + 29 external validations. This wider research can inform new guidelines. Conclusions underscore the value of more research: link.springer.com/article/10.118…
