Everyone who performs thoracenteses ought to burn this image into their memory. Great reason to *always* look with Doppler!
(Just the words “tortuous intercostal” ought to raise *your* HR by a few bpm)

PSA: this is why we ALWAS scan with colour doppler (or CPD, in this case) to look for vessels prior to thoracentesis. Indicator is toward patient's head, so the vessel is running on the superior aspect of the rib --> right where you'd normally poke #POCUS #MedEd

Actually it has been studied and giving contrast doesn't make the AKI measurably worse.
