The beauty of this framework? Stage names stay consistent across sports. The drill content changes. Football, soccer, basketball — same stages, sport-specific execution.

🟦 Stage 5 — Practice Simulation Modified practice, realistic work:rest, tactical demands, training loads approaching competition density.

🔷 Phase 5 — Practice Density Repeated-effort blocks and tactical sport integration that match real competition demands. When late-stage rehab looks like "more advanced drills," it's not periodization. It's improvisation.

Rehabilitation should be periodized. Most of the time, it isn't. Periodization means the systematic manipulation of training variables to build specific qualities in a planned sequence. In late-stage rehab, a practical force-to-performance sequence looks like this:

What's the hardest return-to-sport conversation you've had to have with an athlete or their family? "You passed the tests, but you're not ready." "The surgeon cleared you, but I need more time." "Your numbers look good, but your movement tells a different story."

Claude + Amazon KDP = $8,400/month. I compiled all my personal AI eBook publishing prompts into a 53-page Claude playbook. I've shared this with a banker, he's now making $60,000/year. I’m planning to charge $99 for this soon, but today, it’s FREE. Like + Comment "Doc” I’ll send it to you. (Follow so I can DM) ⏳ Taking this down in 24 hours.

This is why the Panther Symposium recommends combining symmetry with ABSOLUTE capacity thresholds — not replacing one with the other. Stop using LSI as your only readiness signal. It was never designed to carry that weight.

if the uninvolved side has also lost strength, you're inflating the score. An athlete at 85% LSI who had 100% uninvolved-side capacity isn't the same as an athlete at 85% LSI whose uninvolved side dropped to 85% of its pre-injury level. Both score the same. Neither is the same.

The most dangerous words in sports rehab: "Their symmetry looks good." Here's why: When an athlete deconditions post-injury, BOTH limbs lose capacity. So when you calculate a limb symmetry index comparing involved to uninvolved,

Incorporating self-report tools isn't soft. It's science. KOS-ADLS. IKDC. ACL-RSI. Global Rating Score. These don't replace your clinical eye. They sharpen it.

We measure quad strength to the decimal. We should be measuring fear with the same rigor. Psychological readiness is one of the most underused domains in return-to-sport assessment and research suggests it may be one of the most predictive.
