The correct answer: Imipenem
It is associated with high emergent resistance among them, and that’s why it is combined with Cilastatin and Relebactam as (Imipenem Cilastatin-Relebactam combo). It is mainly indicated for DTR cases.

Do not be confused w the approach to treat neutropenic patients with bacteremia as they are considered a high risk group of resistance—> mainly started with combined Abx

In general, if you are targeting P.aeruginosa as an empirical therapy —> try to choose beta-lactams group - unless there is a history of high resistance before with a sepsis like picture - —> FQ is the 2nd choice 👍🏼✅

عاد تدروا ان نزل واحد جديد نسبيا في الساحة اسمه Delafloxacin
1️⃣ It has better lung penetration
2️⃣ It has better biofilm penetration.
3️⃣ It can work better than others in low pH.
4️⃣ it has safer medical profile in terms of (long QT & Tenditis).

لكن اذا عندك عدو و تبي تحاربه ما تطلع كامل اسلحتك له؛ لهذا ما نستخدم:
Ceftazidime avibactam
Ceftolozane tazobactam
الا لو كان عندنا انواع pseudomonas مقاومة للCarbapenems
وندخّر الاسلحة النووية كحل اخير اذا مانفعت جميع الاسلحة, مثل:
Polymyxin C
Colistin
Cefiderocol
3rd generation:
1- Ceftazidime-avibactam.
2- Cefoperazone.
Miscellaneous cephalosporin:
1- Cefiderocol.

معلومة اليوم الصادمة .. الكل يفكر ان:
4th generation of cephalosporins هو الوحيد الي يغطي بكتيريا P.aeruginosa، ولكن الحقيقة اللستة تطول .. 🤣

بل هذا الاسلوب في العلاج له فوائد محتملة كثيرة، مثل:
١- يقلل من نسب الوفاة مقارنة بالجرعات المتقطعة.
٢- جرعة اقل من الدواء بنفس فعالية جرعة متقطعة عالية، بل افضل في بعض الدراسات.
٣- يقلل من sub-MIC of antibiotics والي بدوره يقلل من مقاومة البكتيريا للمضادات خلال العلاج او بعده.

تبدا تستوعب امور كثير بعد ماتعرف ان الbeta lactam يعتبر time-dependent killing وليس dose dependent killing وهذه لها اثر كبير في علاج بعد انواع البكتيريا، مثل:
P.aeruginosa لانهم يكونوا حساسين لبعض المضادات ولكن الMIC/minimal inhibitory concentration عالي لهم
من تخلص سرجري و بيديا المفروض يسلموك وثيقة التخرج .. سقط جسر الميدسن بل يمكن واحد من اخف الروتيشنز عندي

طبعا هذه هي الخيارات في حالات معينة ولكنهم لا يعتبروا العلاج الرئيسي لهذا المرض إنما نستشهد بهم على سبيل المثال وليس الحصر

Neutrophil & Phosphodiesterase4 inhibitors?
احنا نعرف ان PDE4 يعملوا ع منع تكسر cAMP
فوجدت الدراسات أنه يعمل كanti-inflammatory molecule ويقلل من افراز:
- TNF-alpha
- IL-8
الي هم السبب الرئيسي في تواجد neutrophils and macrophages
ومن هنا طاح الاختيار على PDE4 👍🏼
نعطي مثال ف علاج حالات COPD:
اذا كان eosinophil count اقل من ١٠٠:
- Phosphodiesterase-4 inhibitors is a valid choice.
واذا كان eosinophil count اكثر من ٣٠٠:
- Inhaled Corticosteroids
- IL-4 receptor inhibitors
