The answer is: patent foramen ovale (PFO)! The foramen ovale is patent throughout embryonic and fetal life in nearly all hearts. Premature (intrauterine) closure, by fusion of the valve to the limbus, may result in hypoplastic left heart syndrome. During the first year after birth, the same fusion process occurs in about two thirds of hearts and structurally closes the foramen ovale. In the remaining one third, functional closure is achieved because left atrial pressure exceeds right atrial pressure and thereby forces the valve against the limbus. This is different from an atrial septal sefect (ASD) which occur within the true interatrial septum (secundum type) or within neighboring structures that then result in an interatrial communication (primum, sinus venosus, and coronary sinus types). #CardiacPath #ThoracicPath

You all are spot on! The correct answer is PAPILLARY CARCINOMA, INVASIVE ENCAPSULATED FOLLICULAR VARIANT. The first image demonstrates this encapsulated tumor is composed of follicles throughout and displays a focus of mushrooming capsular invasion (top). The second image demonstrates cytomorphology at higher magnification, and vascular invasion in the form of a tumor thrombus hanging in the lumen of an intracapsular vessel.
