A day in the field today - spending time with our Self Management Facilitators in Derby. Seeing first hand the impact they are making for patients, as well as reducing demand on overstretched community and ward teams was the highlight of my summer so far @Emi1yBriggs @DCHStrust

Incredible work from our Rapid Response teams during May to meet UCR demand. Over 82% of patients were seen within 2 hours. Hugely significant for not only patients but also reducing pressure on emergency and urgent care services. Hard work from so many making a difference
