Happy 😊 to see our recent work featured today in ASN In the Loop. @ASNKidney Kidney involvement in primary Sjögren’s syndrome remains underrecognized yet clinically meaningful. In this biopsy-confirmed cohort from Mayo Clinic (2012–2025), we examined clinicopathologic patterns, treatment response, and long-term outcomes in patients with renal involvement. @jingmiao2019 @MayoClinicNeph Several findings stand out: • Tubulointerstitial nephritis was the most common lesion • Baseline CKD was highly prevalent • Immunosuppressive therapy was frequently used, with substantial rates of complete recovery • However, long-term risk remained significant, with nearly half reaching a composite outcome of CKD progression, kidney failure, or death • Higher baseline creatinine, proteinuria, and segmental glomerulosclerosis were associated with poorer recovery and worse prognosis Renal involvement in Sjögren’s syndrome is uncommon, but when present, it carries important prognostic implications. Early recognition, biopsy-guided classification, and risk stratification matter. Grateful to collaborate with an outstanding team on this work. Bottom line: MCCS and NIH-CI are comparable and add prognostic signal beyond clinical data—with IF/TA doing most of the heavy lifting—and performance was consistent in Class V LN. MCCS may offer a practical edge by aligning chronicity grading across glomerular diseases.
