@NicUof We are currently looking at this. It’s an interesting question- does the assumption that stopping caffeine is a basic part of the discharge process lead to delays or even harm from intermittent hypoxia? Why not stay on it for longer?

Frequent early-life apnoeas and periodic breathing may contribute to differences in neurodevelopmental outcomes between healthy moderate-late preterm and term infants. This recently published cohort study analysed temporal trends in oximetry indices. Results showed increased intermittent hypoxaemia frequency and severity in moderate-late preterm infants (born between 32+0 weeks and 36+6 weeks) compared with term infants during the first five postnatal weeks. Despite comparable mean oxygen saturations, moderate-late preterm infants’ more frequent and severe desaturations suggest an under-recognition of respiratory instability if relying solely on mean saturations. So I had an idea. Hospitals should be required to give every new parent a pamphlet on baby safety. It should include everything from “don’t leave your baby on a changing table” to “don’t give honey to children under the age of two,” to “don’t give babies water.” There’s a lot of basic stuff that people don’t know, and aren’t necessarily going to be taught, even in a parenting class.

I think we often do not give enough sodium to our premature babies in the NICU. TPN orders say 2-4mEq/kg/d is enough. This RCT won't change minds that it should be higher, but there was faster growth for the first few weeks on protocol of more Na. publications.aap.org/pediatricsopen…
