La enfermedad de Parkinson es una paradoja biológica: una dolencia pequeña en su origen —la pérdida progresiva de neuronas dopaminérgicas en la sustancia nigra— con consecuencias enormes para el cuerpo y la mente. Es una enfermedad del movimiento, sí, pero también del tiempo, del ánimo y de la identidad. Síntomas motores. El Parkinson se manifiesta con una coreografía lenta y persistente: bradicinesia (la dificultad para iniciar y ejecutar movimientos), rigidez muscular, temblor de reposo y alteraciones posturales. No es el temblor lo primero ni lo más grave; lo más devastador es la lentitud, esa resistencia invisible que el cuerpo impone a cada gesto cotidiano. Síntomas no motores. Mucho antes de que el temblor aparezca, el cerebro ya avisa. Trastornos del sueño REM, depresión, ansiedad, hiposmia, estreñimiento, fatiga, dolor, disfunción autonómica y, en fases avanzadas, deterioro cognitivo y demencia. Estos síntomas recuerdan que el Parkinson no es solo una enfermedad dopaminérgica, sino un trastorno sistémico del sistema nervioso. Epidemiología. Afecta a cerca del 1% de la población mayor de 60 años, y su prevalencia aumenta con la edad. Es ligeramente más frecuente en hombres y, con el envejecimiento poblacional, se ha convertido en una de las enfermedades neurológicas de más rápido crecimiento a nivel mundial. Etiología. La causa no es única ni simple. El Parkinson surge de una interacción compleja entre genética y ambiente. Mutaciones en genes como SNCA, LRRK2 o PARKIN conviven con factores ambientales —pesticidas, metales pesados, toxinas— y con procesos celulares universales: estrés oxidativo, disfunción mitocondrial, alteraciones en la autofagia y la acumulación patológica de α-sinucleína en los cuerpos de Lewy. No es una falla aislada, es un sistema que pierde el equilibrio. Tratamiento. No existe cura, pero sí estrategias eficaces para aliviar los síntomas y mejorar la calidad de vida. La levodopa sigue siendo el tratamiento más potente, acompañada de agonistas dopaminérgicos, inhibidores de MAO-B y COMT. En casos seleccionados, la estimulación cerebral profunda modula circuitos alterados con notable eficacia. A esto se suman ejercicio, rehabilitación, atención a la salud mental y un enfoque integral que recuerde que tratamos personas, no solo neuronas. El Parkinson nos enseña una lección incómoda y fascinante: que la complejidad del cerebro humano puede empezar a resquebrajarse por la pérdida de unas pocas células, y que entender esa fragilidad es una de las grandes tareas de la biología contemporánea 🧠✨
