Magnetic Resonance Imaging–based Biopsy Strategies in Prostate Cancer Screening: A Systematic Review sciencedirect.com/science/articl… Systematic review evaluated the benefit–harm balance of incorporating magnetic resonance imaging (MRI) into #ProstateCancer (PCa) screening as either a first-line test or a second-stage test after PSA >3 ng/ml. Across five first-line and four second-stage studies, MRI strategies were compared for cancer detection, biopsy avoidance, and histological outcomes. First-line MRI identified roughly twice as many clinically significant (GG ≥2) cancers as second-stage MRI but left a higher proportion of men MRI-negative, while second-stage MRI markedly reduced biopsy rates and increased the detection ratio of GG ≥2 versus GG 1 cancers with both targeted and systematic biopsies. Oncological Safety of MRI-Informed Biopsy Decision-Making in Men With Suspected #ProstateCancer jamanetwork.com/journals/jamao… This cohort study assessed the safety of omitting prostate biopsy (PB) in men with suspected prostate cancer who had negative multiparametric MRI (mpMRI) results. Among 593 men studied, 48% had negative MRI results, and 86% of these men avoided biopsy over a 3-year period with ongoing clinical monitoring. After 3 years, only 4% of men with negative MRI results were diagnosed with clinically significant prostate cancer (csPCa). The findings suggest that prebiopsy MRI has a high negative predictive value, allowing men with negative MRI results to safely avoid PB when they are closely monitored. This approach supports the use of MRI to reduce unnecessary biopsies without compromising cancer detection. @Charlie_Charite @AsbachP @IvoSc @CashHannes
