The incidence of Barrett’s oesophagus is increasing in the UK, particularly among men less than 40 years old, even when detection bias is accounted for. One study that used a population-based register found the annual incidence rate rose by 159%, from 23.9/100 000 during 1993 to 1997, to 62.0/100 000 during 2002 to 2005, far exceeding the increased number of endoscopies performed over the same period. PMS is also often trivialised and many women struggle to access appropriate help and support. Some individuals will have predominantly psychological, predominantly somatic or a mixture of symptoms, but it is the timing, rather than the types of symptoms, that supports a diagnosis of PMS. In order to differentiate physiological menstrual symptoms and other conditions from PMS, it must be demonstrated that symptoms cause significant impairment to the individual during the luteal phase of the menstrual cycle (ie in the two weeks before menstruation). The severity of symptoms is judged according to the degree of interference with day to day activities and relationships.
