The Step 1 pass rate for IMGs has dropped from 82% to 72% in just 2 years?! 😳 And from 96% to 92% for 🇺🇸 students! What do you think is the reason? Source: USMLE.org

@jbcarmody Substantial drop in Step 1 scores seems like an argument for them to not be pass/fail.

I doubt we’ll ever see the score distribution for a pass/fail USMLE Step 1. However… …from data revealed in the USMLE cheating scandal, we do know that the mean Step 1 score dropped by nearly 1 SD after the test went pass/fail (from 235 in 2020 to 219 in 2023).
Nicholas Young, MD FACP@nickyoungMD

@jbcarmody Are they planning to release the score distribution curve post-P/F? It would be nice to know the affect on the curve as a whole as well.

@malkeasaad You leave out the fact that the US is extremely litigious and that no malpractice insurance company will protect you for performing outside scope of practice.

Did you know that in the United States, if you’re licensed to practice medicine in a state, you can practice in whatever area of medicine you like, even if you’re not trained in that?? For example, a pediatrician can do a plastic surgery operation like below!!

@jbcarmody Make the Match ranked choice voting + lottery. Students list preferred specialties in rank order. Specialties fill available slots with top scores. If not matched to specialty, then drop down to next unfilled specialty. Lottery distributes student within specialty. Interviewless.

If anyone else has wondered about questions like these… I’ve got a whole 6-part video series for you, covering why we have a match, the design of the algorithm, the antitrust suit against the NRMP, and whether we’d be better off without a match. (link ⬇️)

@jbcarmody @jbcarmody Why is there a match system in place anyways? Why can’t residency spots work like medical school spots - you interview and then get as many offers; then, you choose which one is best for you

Less than one day into The Waiting and already my inbox is full of messages asking: “Why does it take so long to get Match results???” Last year, I made a video answering this question (and, as an added bonus, counted down history’s Top 5 Match Day Mishaps). ⬇️

@jbcarmody One way to control application costs is to reduce applications. An application cap for all should reduce costs to applicants and focus applications to programs in which they are actually interested. This will stop interview hoarding. This is good for applicants and programs.

Starting next year, ERAS will give a 60% fee reduction to certain financially-needy residency applicants. Look, lowering ERAS fees is a good thing. But for all the publicity it’s received, the actual number of dollars we’re talking about is… not many. (🧵)

@jasonryanmd One metric that residency programs are judged by ACGME is the percentage of their graduates that pass specialty board exam on first try. Step scores highly correlate with board exam success.

Questioner at AAMC session on Step 1 says students only focus on exam and aren’t getting “educated” any more on how to really be a doctor. I don’t think many people understand how crazy this test is forcing students to focus on passing.

@jasonryanmd "Passing" Step1 is a pretty low bar and was enacted to de-stress this test. This just makes Step2 even more high stakes.

@jasonryanmd What metrics do you suggest be used to evaluate applicants? The metric should be validated as relevant to success as a resident. Applicants are competitive, and I believe any metric used would become high stakes. Step2 Score and Advanced Degree have been validated in studies.

@minhaj_areeba When candidates apply to 40 residency programs, programs need to have an algorrhythm for interviewing high likelihood candidates. If applications per candidate capped at 10 programs, then the interview math would change.

@AmnaFarooq7 If you have high board scores or did a sub-I at the program, then signals are important. If you have low board scores, and you have never visited the program or done a sub-I at the program, then the signal may actually act as an anti-signal (your program is my safety).

I am not getting the concept of signalling The programs I signalled are not considering the signals and the programs I did not signal are considering the signals 🤔 How would I have known this before ?everything here is so confusing 💭 I need more signalssss #Eras #Match2024
