Secretory/lactational change can cause nuclei to look quite atypical and abnormal, with irregular nuclear membranes, enlarged nuclei, and prominent - sometimes multiple - nucleoli. Cytoplasmic vacuolization is a clue to the secretory nature of the cells. Remember that nuclear atypia without an indication of abnormal proliferation is insufficient for a diagnosis of atypia. The architecture in this case is beautifully acinar. In fact, this was from a tubular adenoma that grew in size during pregnancy. Trainees: Always take isolated atypical cells or atypical cells with a normal architecture with a grain of salt. #breastpath
