@coollyndz Yes and no. It’s not about sustaining PAO2 to spend the rest of the flight at cruising altitude (you’re right, you're pissing in the wind at 38000ft), it’s about prolonging TUC just long enough to allow the pilot to descend to a more physiologically conducive altitude.

In Palma awaiting the aeroplane safety brief so I can stand up and give them a lecture on the alveolar gas equation and why an o2 mask for a loss of altitude is useless.

@CatrionaDavis I will defer to a native Gàidhlig speaker but is it perhaps more like “wee mousey”?

Hello! I’m looking for a GP locum to work 3 days a week for 6 weeks in July and August 2023 in THE best practice in Scotland (Dunkeld), to allow me to have a sabbatical. Please share, and get in touch for more details.

@nickpheath @StephenHearns1 Leave clothes on but ensure pockets empty and zips are out of the way for binder. Don’t want to risk inducing hypothermia in your potentially bleeding patient.

Hi @StephenHearns1 I’m doing some work with ski patrol at the moment and wondered what you advise in MRTs about splints and pelvic binders next to skin vs clothes in extreme cold. Any links or advice would be much appreciated

Any other prehospital services using measuring K+ & oesophageal temps on scene for prognostication using HOPE score please get in touch. Keen to assess how widespread use is for exposure hypothermia, avalanche & water incidents.

Got some new gloves at work that will finally end the scourge of fentanyl exposure. We’ve lost too many good staff to accidental overdose through skin exposure. #fentanyl #ppe

Are there any pre hospital colleagues who would consider using/already do use nebulised salbutamol in a highly suspected hyperkalemia? Suggested it last night and I was stared at like I had suggested we IO someone in the face.

@clare_eliza @DrBenLovell There’s a trial that showed worse outcomes with blood resuscitation if Hb >70/80 though! I don’t know whether it was subdivided into the oozy ulcers and the OH GOD IT’S EVERYWHERE variceals as the latter may behave more like a traumatic bleed.
