This rockstar attitude to resus is sad. "Shame" that he got ROSC so couldn't try DS defib. Whacking the LUCAS and pads on an elderly nursing home resident is not in anyone's best interest. No thought of weighing up the harm Vs benefit and giving this patient a good end of life.

1/ Wild cardiac arrest with #POCUS playing a major role An elderly male was sent from the nursing home in cardiac arrest. Per the prehospital report that we got, the patient was in vfib and refractory to 5 defib attempts. The patient got amio as well as several doses of epi.

@BennyGailius @em_bulance Urm just of the top of my head- patient with confirmed Stemi- brave move to convey with no defib attached. I’d rather they sat with us (trained in bls) then got in a cab with no support… doubt I’d sit in waiting room though 😂

A topic I’m curious to get others opinions on. When GP’s call 999 to transport a patient, do you think leaving said patient in the waiting area unattended is safe and reasonable? Or would you say that if this patient needs an emergency ambulance they should be monitored?

@kieranfbUK @Nat_O_Byrne @em_bulance Absolutely and I personally have been guilty of criticising ESPECIALLY when a student and soon after qualifying but now I’m definitely more understanding of the pressures and how everything can’t be ideal it can be risk mitigated as much as possible

@BennyGailius @Nat_O_Byrne @em_bulance Agree. Should defo hand over to crew on arrival too. Just feel some are easy to criticise GPs (not anyone in this thread necessarily) whilst not appreciating the bigger picture.

@LouiseWackett1 @em_bulance Not necessarily. E.g. Chest pain would possibly require ongoing monitoring whereas #ankle or #foot with inability to get to centre where X-Ray and treatment can be provided but no alternative transport available wouldn’t need ongoing monitoring

@kieranfbUK @Nat_O_Byrne @em_bulance NHSE advises that HCP L1/2s requests should have the HCP remain with the patient if possible with crews receiving a handover at scene. I completely agree with having a GP carry on assessing others but there should be clinical supervision if a 7/18 minute response is required

My own opinion is that there’s a reason that a HCP request has been made and there has to be justification for that decision but unfortunately we don’t have enough feedback (due to time/presentation) as crews to understand what the rationale is

@ambogemma @em_bulance Have you got any examples of presentations that some patients may have that are f2s in WR but need ambulance conveyance? There’s many frustrations from both crews and pts if they’re waiting 1+ hours then they could’ve had a taxi transport them instead

@statto95 @NJL_Blancq @loddyy24 @studentparabeck Some trusts provide their uni students with full uniform providing a uniform workforce whilst ensuring the correct safety standards are met by the uniform. I had ALL my student uniform issued to me by the trust and my uni clothing was to be work in labs or hospital placement

@NJL_Blancq @loddyy24 @studentparabeck You are aware that, as I've stated, I'm talking External/Uni students not Internal/Trust students yeah.... Or did you not read that part.... If you're a Trust student same uniform & logo. If you're an external student you need external logo not NHS logo....
