@LipidSniper @PeterAttiaMD The ratio should be customized for each patient; in general you decrease the dose of LT4 by 25 mcg and introduce LT3 5 mcg. Adjustments follow based on symptoms; TSH levels must remain within normal range.

In your book you cite this but also say to start with 14:1-20:1 ratio which is confusing. Please clarify. Thank you

@Bianco_Lab Fantastic paper. Why do some patients on T4 only with optimal TSH and Free T4 end up with below range T3? Is it due to deiodinase polymorphisms or some other factor? And does exogenous T3 correct the dyslipidemia? Thank you for groundbreaking work and a fantastic book. Cheers!

We followed 105 patients before and after developing hypothyroidism (on LT4). Those patients with lower T3 were at risk for dyslipidemia (47.5% vs. 27.3%); even after adjusting for confounders (adjusted OR: 3.6). Monitoring T3 levels should be considered. endocrinepractice.org/article/S1530-…

@EnajaratG The findings are valid only for patients with hypothyroidism. The association between treatment that contains T3 with reduced cases of dementia is very significant. There are between 15 and 20 million patients with hypothyroidism in the US.

Over a 20-year follow-up, patients receiving combination therapy (LT4 + T3 or DTE) had a 27% lower risk of dementia and a 31% lower risk of mortality compared to those receiving LT4 alone. Adjusted Cox proportional hazard models confirmed the results.
