@zach_london I think THIS last sentence is the heart of it. Your WHYs make a lot of sense. I haven’t seen many WHY NOTs or support for the way it is currently. Is there rationale from fellowship side or just an antiquated “way it’s always been” carry over?

Lest it seem I'm picking on one specialty, I will mention that MS is not the only field with a problematically early timeline. Vascular neurology and epilepsy are almost as early, with several others close behind. I'm still waiting to hear a good explanation for this.
