@EMBoardBombs CXR has poor sensitivity for PNA, detecting pulmonary opacities only 43.5% of the time (pubmed.ncbi.nlm.nih.gov/23083885/). A normal CXR doesn’t rule out CAP. Clinical judgment should drive diagnosis and treatment—relying on imaging alone risks missed cases and delays in care.

Diagnostic criteria for CAP: fever, respiratory symptoms & imaging evidence of an infiltrate. If you don't have these, don't prescribe antibiotics for the indication of pneumonia.

a terminally flawed review of neuroleptic malignant syndrome was published in the NEJM yesterday: nejm.org/doi/full/10.10… - no mention of ice-water immersion for hyperthermia - patient in fig. 1 is being cooled with an arctic sun - acetaminophen recommended for hyperthermia

1/ Today's 🧵is about "VT" versus "SVT with aberrancy". The aim of this thread is to provide basic tips on how to apply some of the most used criteria that might be helpful in diagnosing VT. #CardioTwitter Note that the following features are suggestive of VT, but their absence does not exclude VT. @ekgdx

VFib and Arrythmias In zones of differential excitability, some areas may respond quickly to an electrical impulse, while others respond more slowly or not at all. This can lead to uncoordinated contraction of the venticular muscle (resulting in VFib or PEA). 14/15
