Thankful for the opportunity to represent the "V-RULES real-world registry" of patients treated with CPX-351 for AML (t-AML or AML-MRC, per its FDA label) at #ASCO25, on behalf of a great team of investigators across 10 sites! This analysis confirms randomized phase 3 data demonstrating efficacy of CPX-351 in t-AML/AML-MRD and provides insights about efficacy and outcomes in patients age <60 who were not included in the phase 3 trial. This work confirms that CPX-351 should remain the standard of care therapy for patients with t-AML or AML-MRC who are fit for intensive induction. In V-RULES, the mOS in patients aged <60 was 17.8mos, vs 10.6mos in those >=60 years old (which compares favorably to 9.56mos in the phase 3 trial, which enrolled only patients age 60-75). Analysis landmarked from transplant continues to suggest sizeable long-term benefit in patients who receive CPX-351 treatment before alloHSCT, compared to expected results with standard 7+3 or similar induction regimens, also consistent with the phase 3 trial results. Unfortunately outcomes remained poor in the subgroup with TP53m AML, with median OS of 5.3mos, consistent with prior trials. New approaches are desperately needed for TP53m AML.
