We don’t think enough about healthcare burden. 🪨 Post-transplant cyclophosphamide (PTCy) was a revolution, not only because it worked but because it was effective, inexpensive, and easy to deliver. It removed complexity from a process that desperately needed simplification. So we adopted it. Bone marrow grafts are associated with less chronic GVHD, yet we often prefer peripheral blood stem cells. That choice is not always about outcomes but about the greater burden on the donor and the transplant center when using marrow. We talk a lot about efficacy but not enough about ease. Making the lives of our patients and healthcare teams simpler, less friction-filled, and more sustainable should be part of how we define success. 🥰 Where should we focus next to reduce healthcare burden while improving outcomes?
